CARES Programs include...
- Service coordination, including networking among mental health providers, medical providers, financial providers and more
- Social and support opportunities across the state
- Housing program through one housing facility and scattered sites
- Emergency financial assistance, budgeting feedback and food vouchers
- Resource bank for donated goods and services
- Stigma program for educating employers and community about HIV facts, providing advocacy for compassionate care
- Healthy Cities program of donated organic vegetables during the summer
- Benefits counseling (including access to Medication Assistance, Medicare, Medicaid, PATH, and other public assistance programs)
- Harm reduction and HIV co-infection risk counseling
- Rides to medical appointments and sometimes chores and shopping
- Information and referrals
- Volunteer coordination
- Advocacy with other care providers
- Coordination of end of life services and hospice
Staff roles may include (but are not limited to):
- Monthly meetings for support, information, referrals and medical care plan check-ins
- Information and referrals for nearly every aspect of care and well-being
- Offering rides to critical appointments and life functions
- Volunteer coordination to supplement care and case management
- Accessing federal or private financial assistance if needed
- Harm reduction and HIV co-infection risk counseling
Staff roles do NOT include (but we may still offer help through referrals for):
- Moving or transporting clients' property
- Mental health therapy
- On-call crisis/emergency services
- Immediate shelter for homelessness
- Substance abuse treatment
- Property management of our supportive housing program
- Financial assistance for complementary therapies
- Financial assistance for gym memberships
- Direct employment training
- Direct budgeting
